Medication on Trips
If your student takes any regular medication, or if you anticipate your student needing any type of medication on any out-of-town trip we take, your child’s physician must complete the medication form linked below. This includes prescription and over-the-counter medications. If Buchanan already has this signed form on file for the current school year, you do not need to have it completed again, unless there have been changes.
Even if your student doesn’t take any regular medication, we strongly advise having your doctor complete the form indicating that a few basic over-the-counter medications are okay in case of a headache, cold, fever, etc. This form can often be completed without having to go to an appointment. Reach out to your doctor for more information.
Please note: Your doctor can indicate that the medication is ‘self-carry’ and will be transported and administered by the student themselves. Any medication not marked ‘self-carry’ by the doctor will be checked in before we depart and administered by an adult throughout the trip.
Once the form has been completed, you can send it to us and we will make sure it gets filed with the school nurse.
