BHS Loan Agreement for Uniform and/or Other Items

Uniforms and/or other related items needed by the student for performances in Buchanan Instrumental Music program will be loaned in cases of financial need. These items are of a specific look and style and must be obtained through the program to ensure they match the rest of the ensemble. Loaned items will be used but in good condition.

“By completing the form below, I agree to the loan of this item(s) to the student listed. I will ensure that the item(s) are properly cared for and maintained. Pursuant to Education Code section 48904, in the event that an item(s) is damaged beyond normal wear and tear, altered, lost, or stolen, I agree that I am financially responsible and will be required to pay for the full cost of the repairs or replacement. The assessed value and/or damage shall be determined by BEC Instrumental Music and/or by the maker of the item(s). I further agree to return the item(s) at any time upon request of the school.”

  • School-Owned Instrument ($ Varies)

  • Concert Dress (Wind Symphony/Chamber Orchestra Only) ($75)
